The 33-year-old started on Friday but played only 23 minutes before being replaced by Mohamed Hany as he appeared to struggle with an injury.
Al Ahly’s medical team has declared Fathi unfit for the return-leg, announcing that he will not travel with the team to Tunisia to play the decisive game on Friday, November 9.
Fathi becomes the third Al Ahly player to miss the game alongside Ali Maaloul and Junior Ajayi, and will be replaced by 22-year-old Mohamed Hany, who performed well in the first-leg.
The former Hull and Sheffield United fullback has played 11 games in this year’s CAF Champions League campaign, missing only two games against Kampala City and Township Rollers in the group-stages.

He has managed to lift the title three times before in 2008, 2012, and 2013, and will hope his side can do it without him next Friday at Stade Olympique de Radès.
The second leg match will take place this Friday, 9 November 2018.
