A turgid first 45 ended on a high note for the visitors as Calvin Bassey pounced at the far post to nod home a flick-on from a right-wing corner.
United toiled in search of an equaliser and pulled level to force extra time when Bruno Fernandes netted his eighth FA Cup goal in his last 12 outings in the competition.
The hosts brought on 17-year-old forward Chido Obi and he had a couple of chances to win it, but Fulham held on to take the tie to what proved to be a triumphant shoot-out.
